If you are a first time patient at our office, you will be given a "Patient History Questionnaire" form so that we can have a better understanding and knowledge of your current and past medical history.  Like most, there is a good chance that you will not enjoy filling out this form.  To expedite this process, we have made available to download a copy of the actual questionnaire form.  Please print out and fill in the necessary sections to the best of your ability and bring it in with you on the day of the eye appointment.  Thank you for taking advantage of this service.
